It's bubbling up slowly as political issue,
but as Marty says, heroin/opiod addiction is hitting every part of society. The visible nonrespectable "drug trade" might still be centered in the big bad cities, but drug use and addiction never was. It shouldn't take epidemics hitting the more respectable members of society for them to be taken seriously as a non-criminal policy matter, but that's the way things work.
